DHS Referral Process
DHS Workers
Our clients have to have been in DHS care and custody for 180 days after their 14th birthday.
Before a youth can be assigned an ILP case manager, they must attend an orientation.
Orientations are given every Tuesday at 4pm, at our office, @ 5605 E. Burnside.
To refer one of your clients to our program you must:

Fill out a Independent Living Program (ILP) Referral Form (AKA: CF 80)

Fax that form to Child Welfare East Branch @ 971-673-2020.

Make Sure Your Client Goes to an Orientation!
We serve youth currently living in Multnomah County
regardless of where their DHS case originates from.
We serve all Zip Codes EXCEPT: 97211,
97212,
97203,
97217 and
97227
If your client lives in one of the above Zip Codes, they would be served by the ILP at Impact NW
